Service Provider Membership Requirements

Please direct all enquiries to 


  • Complete online application form
  • I.D – applicant and employees
  • Policy mandate regarding EAP and wellness services- company profile and organogram
  • CIPC documents
  • Proof of qualifications of main role players in employee assistance or wellness – certificates and CVs
  • 2 References from clients
  • Copy of valid tax certificate
  • Proof of membership of a statutory body of the members to be registered (if any) – e.g. SACSSP or HPCSA
  • Proof of payment (Includeds a non-refunabale administration fee of R200)
  • Proof of chapter attendance – if previously registered with EAPASA
  • Latest EAPA-SA Registration Certificate – If previously registered with EAPA-SA
